By 5pm on 31 July you must have submitted this form along with your CV (if you are applying for scholarships or internships) and the recording of your six minute presentation.

To confirm your entry in the Eureka! Awards you need to submit the name of your presentation and a bibliography of your supporting references.

Information about creating your entry can be found here here. Your video will be submitted through a link you will receive once you have completed this form. You must submit your video by 5pm on 31 July.
